Funded Program:
Diabetes Self Management Education (DSME): This program is intended to increase the number of community members being screened for and receiving treatment for diabetes. Those diagnosed are referred to the American Diabetes Association certified, 10 week series of DSME classes. This program uses culturally competent programming to improve diagnosed patients' diabetes knowledge and disease management skills to stabilize and improve patient blood sugar levels. In the long term Bay Clinic hopes to improve quality of life for patients as well as reduce the rate of diabetes related complications such as stroke, amputation, blindness, renal disease and death in our community. The DSME program is a creative evidence-based approach that supports informed decision making, self-care behaviors, problem solving and active collaboration with group members and the health care team to improve clinical outcomes, health status and quality of life.
Client Outcome Story: "When I started I weighed 220lbs and my AIC was 7.9. I was able to lose about 15 lbs and my latest AIC was 6.5. I have learned how to eliminate the white stuff from my diet: white potatoes, white sugar, white rice, and white flour. I am eating more salads, fish, drinking more water and checking my symptoms regularly and I have stopped eating butter."
